GDA Medical Plan

GDA members and their full-time dental staff members are eligible for coverage under the GDA Group Medical Plan underwritten by Blue Cross Blue Shield of Georgia. The GDA offers a variety of Preferred Provider Organization (PPO), Point of Service (POS), and Health Savings Account (HSA) plans. Prescription Drug Benefits

An annual pharmacy deductible of $200 per individual is part of all plans. The pharmacy deductible applies only to Tier 2 and Tier 3 drugs (not to Tier 1 generic drugs). Enrollees are responsible for the entire cost of a Tier 2 or Tier 3 drug until the deductible has been satisfied. Once the deductible has been satisfied, applicable co-pays will apply. The GDA Group Medical Plan also has a Tier 4 drug plan with specific payment requirements. This tier consists primarily of specialty medications. COBRA Eligibility

All current and future GDA Group Medical Plan enrollees are eligible for COBRA benefits. Click this link to read the COBRA What You Need to Know form. GDA Health & Welfare Plan Document

This document must be provided to each employee in the dental office once they become eligible for the GDA Group Medical Plan, or already are eligible. Included Life Insurance Coverage

All primary medical plan subscribers who are under age 70, actively at work, and not retired, have life insurance coverage. Health Care Reform & Your Medical Plan

The Patient Protection and Affordable Care Act (PPACA) will have significant impacts on all plans that offer medical coverage in 2013 and beyond.
