What is a Smile Kit?
A Smile Kit includes a toothbrush, toothpaste and floss (depending on availability) in a GDA Foundation take-home bag! They are provided to Georgia-based programs and projects that include an oral health education or outreach component.
Who Can Apply?
Your organization is eligible if you are conducting a program or project that includes oral health education and/or outreach and if you are:
- A public charity with a tax-exempt status under section 501(c)(3) of the Internal Revenue Code
- A public or government entity such as a school or health department
- A tribal organization with 7871 tax-exempt status
- A church
- A Give Kids A Smile/Children’s Dental Health Month Program that has registered through the ADA (go here to register.)
Your organization must have a physical presence in the state of Georgia, and the project/program for which you’re requesting Smile Kits must benefit people living in Georgia (with the exemption of mission trips.)
How Do I Apply?
It’s easy! Fill out the application by clicking the button below. Please note that your application must be submitted by the 25th of the month prior to when the kits are needed.*

Your application must be submitted by the 25th of the month prior
to when the kits are needed. You will be notified by email if your request was approved or denied. Pick-up will be available the first business day of the following month. (example: Submission for March 25, 2020 deadline – pick-up is available starting April 6*).
We ask Smile Kit recipients to post about your project or program on social media and tag the Georgia Dental Association (@GADentalAssn) and use the hashtag #GAsmiles.

*Give Kids A Smile requests must be submitted by January 3. Supply pick- Up will begin January 10. Any requests after the January 3 deadline are subject to the 25th of the month prior deadline.
