District and Study Club CE

Local District Dental Societies and Study Clubs are a great option for those seeking continuing education close to home. Societies and Clubs promoting their events are encouraged to include them on the GDA event calendar. 
Local District Dental Societies
Pursuant to Georgia Board of Dentistry Rule 150-3-.09 (2) (a), as tripartite affiliates of the American Dental Association, the Georgia Board of Dentistry recognizes the local District Dental Societies as approved CE providers in Georgia.

As approved CE providers, District Dental Societies are encouraged to create and distribute Verification of Participation forms (CE certificates) for each CE event from their district (and not from GDA). We've made it easy for you. Click here to download your district's Verification of Participation form now.

We also recommend that you distribute this sign-in sheet so you have a record of attendees.

Study Club Requests for CE Certificates
Requests for Verification of Participation forms (CE certificates) for district dental societies and study clubs require considerable administrative effort by the GDA staff. For this reason we ask that anyone requesting Verification of Participation forms follow the procedures below. Requests that do not follow these procedures will not be considered. These procedures ensure we are adhering to ADA CERP guidelines. 

Prior to the Event
Please fill out this form with details about your date, location, presenter, topic, etc. 

During the Event
  1. Use this event sign-in sheet to check in participants at the event. If you are hosting multiple speakers and it is possible attendees may not participate in all, please have attendees sign in for each session. 
  2. When the event or session is over, distribute this Evaluation Form (Word Document - your input is required for the first section) after the event. Again, if you are hosting multiple sessions, please ensure a learning objective is provided for each. 

After the Event
Complete this spreadsheet with information on all CE event participants and email the completed document to Sheila Watson at sheila@gadental.org. CE certificates will be emailed to each participant within 30 days of receipt of this spreadsheet.

IMPORTANT NOTE: For GDA to provide CE certificates for an event the submitting organization must confirm the event meets the minimum standards established in this document.

CE Certificates for Non-members
Verification of Participation forms (also known as CE certificates) are a benefit of GDA membership. Per GDA policy, participants in study club CE events that are not GDA members and receive a GDA issued Validation of Participation form will be charged a $25 fee.
