Share a Smile Program

How it works

To be considered for the program, a community member must be nominated by a volunteer dentist who is invested in treating them.

• Submit an Application. The volunteer dentist completes the Share a Smile Application and uploads the patient’s treatment plan.
• Application review. The Share a Smile Committee reviews the application. 
• Receive Notification. The volunteer dentist and nominee are notified of the application status. 
• Complete the Patient Waiver. If approved, the patient completes and signs a treatment waiver before care begins. 
• Provide Donated Care.  The volunteer dentist provides the approved dental treatment. 
• Submit Post-care Information. The dentist completes a post-care form and submits before-and-after photos and eligible lab expenses for reimbursement, up to $1,000.
